Salary: £33,149 to £39,528 per annum (pro rata, dependent on experience)

Hours: 30 - 37.5 hours per week

We are seeking a compassionate social worker to join our multi-disciplinary community team providing palliative care to adults in Brighton and Hove, Peacehaven and Newhaven through a range of services, including In-Patient, Community and Day Services.

We are either seeking to recruit someone interested in moving their career towards palliative care or a postholder with experience of palliative care.

You will be confident in working with emotional and relational complexity and risk in a multi-disciplinary context; working alongside other colleagues in the Patient & Family Support Team to deliver a seamless psychosocial offer to patients and their significant others.

 Main duties will include:

  • To provide emotional and practical support to patients and their families
  • To co-ordinate the work of the Martlets Community Volunteers Visiting Service
  • To undertake needs assessments and client reviews
  • To provide short term support for the bereaved
  • To provide patients and their families with information about employment, housing and legal issues

Key Skills required:

  • Minimum of 5 years recent social worker experience
  • Social Work qualification CQSW/Dip/BA/MA Social Work
  • Current registration with the HCPC
  • Excellent written and verbal communication skills
  • Experience of working with adults with acute or chronic health problems
