Martlets provides a variety of care and services for those living through a terminal illness in and around Brighton & Hove.

How to refer

There are 2 main routes to accessing our services.

Route 1

Referrals can also be made directly to Martlets for:

Our Supportive Care and Outpatient Services for patients who:

  • Have been diagnosed with a terminal illness
  • Are living in Brighton & Hove, the Deans or the Havens

Our Bereavement Services for the families and close friends of patients who:

  • Were cared for by Martlets in some capacity such as within the In-Patient Unit, through the Palliative Care Partnership, through Hospice at Home or through our Supportive Care and Outpatient Services 
  • Are living in Brighton & Hove, the Deans or the Havens

Referrals will be accepted from Doctors, Registered Nurses, and Allied Health professionals. If you wish to self-refer talk to your GP first.

To refer to Bereavement Services please call 01273 273400 and ask to speak to a member of the Bereavement team or email [email protected] 

 

Route 2

Patients can be referred via the Sussex Palliative Care Partnership- which is a combination service providing care and advice from specialists working across the NHS and Martlets.  

You can contact the Sussex Palliative Care Partnership through our Hub phone number: 01273 964164. This number is only for use of healthcare professionals and patients/carers who have had the number specifically given to them.

After the referral to Sussex Palliative Care Partnership, there can be onward referral can be to one of the following services:

Our Respite Service for patients who:

  • Have a named carer
  • Consent to referral for respite
  • Are deemed suitable for respite using the GSF Prognostic Indicator guidance

The Community Palliative Care team (CPCT) for patients with…

  • Locally advanced/metastatic cancer with complex physical or emotional problems associated with their illness
  • Other life-limiting illnesses with complex end of life care needs for symptom management, decision-making and/or advance care planning, psychological and emotional support

Our Hospice at Home team ([email protected]) for patients who:

  • Are in their last few weeks of life
  • Require short term crisis intervention

Please ensure consent has been obtained from the patient prior to referral and that their GP has been informed.


Our Referral Criteria

Criteria for Referral Route 1

The referral criteria for our Supportive Care and Outpatient Services differs to that for our Bereavement service

Supportive Care and Outpatient Services criteria:

  • Patients receiving these services must have been diagnosed with a terminal illness
  • Only family and carers of those patients diagnosed with a terminal illness may receive these services
  • Patients and family/carers must live in Brighton & Hove, the Deans or the Havens

Bereavement Services Criteria:

  • A loved one was cared for by Martlets in some capacity such as within the In-Patient Unit, through the Palliative Care Partnership, through Hospice at Home or through our Supportive Care and Outpatient Services 
  • Family or close friends of patients who are living in Brighton & Hove, the Deans or the Havens

Criteria for Referral Route 2

Patients must be over 18 years old and...

  • Be registered with a GP practice in or around Brighton and Hove
  • Have a life-limiting illness requiring specialist palliative/end of life care
  • Be on the GSF (Gold Standards Framework) register and/or with a life-limiting prognosis

We accept Referrals from:

  • Hospital Macmillan Nurses
  • Oncology teams
  • GPs and IPCTs
  • The Nevill Hospital
  • Community respiratory/heart failure teams
  • Martlets IPU
  • Neurology teams
  • Short term care and community rapid response teams
  • Healthcare professionals
  • Patients previously known to the Palliative Care Partnership caseload can re-refer themselves
