If you've chosen to receive end of life care at home we can help give you and your loved ones exceptional care and support. We provide the high level of nursing care you’d find on the ward, in the comfort of your own home. Our experienced team will make visits and assist with pain management, symptom control and emotional support, for you and your family. Your District Nurse, your GP, your hospital specialist or our clinical team at Martlets will refer you to the Hospice at Home team when it's needed. Speak to your GP if you think Hospice at home may be able to help you. We’re part of the Palliative Care Partnership (PCP) – a combination service of specialists who provide support, care and advice. Once you’re in our care we’ll make sure that each member of the team, and any healthcare professionals you use regularly, are updated. How much does Hospice at Home cost? Hospice at Home care is free. Should you require longer term domestic and medical care at home, Martlets Care agency can provide these services at a competitive rate. All profits from Martlets Care are re-invested in the charity. Who can access Hospice at Home? Our care is free to people who are over 18 and whose GP surgery is based in Brighton & Hove, the Deans or the Havens. Please see the below map for our service area. We’re here to provide care, compassion and support for people with cancer, heart conditions or neurological illnesses such as motor neurone disease.
