When patients are discharged from our In-Patient Unit many report feeling quite vulnerable. Our Volunteer Discharge Buddy Service aims to help patients feel more secure as they return home by: 

  • Maintaining patient safety 
  • Restoring confidence and independence for patients and family members 
  • Supporting the community teams 
  • Preventing unnecessary re-admissions 

"My Discharge Buddy and the live-in carer worked together to make it as easy for me as they could. I loved being at home again, but couldn't have done it on my own. The Discharge Buddy gave my carer the confidence to sort things out for me - like prescriptions and appointments, so I didn't have to worry."
