Often, being admitted to a hospice can be a difficult and worrying time for patients, family and friends. Within the Hospice the patient and family support team works closely with the nursing and medical teams to provide a range of different types of support to help patients and their families cope with the emotional, psychological, social and spiritual impact of living with a life threatening illness and preparing for death.

Who we are


The team includes social workers, counsellors, hospice discharge planning co-ordinators and a Hospice Chaplain.

What we do


Our social workers provide support to help patients and their families talk in confidence about problems. This includes advice and advocacy regarding practical matters such as arranging care services, housing, financial and other concerns; welfare benefits advice; emotional and practical support for carers as well as advice and information on how to support children.
Our counsellors are available to provide emotional and psychological support in the form of counselling for patients, their families and children, both during their time at the Hospice and after their bereavement.
Our discharge planning co-ordinators are available to help make arrangements for our patients to return to their homes after staying in the hospice following a planned admission. They are also able to plan and liaise with other agencies to enable a family to care for a loved one at home at the end of life if this is required.Our experienced, non-denominational, lay chaplain is here to support, listen, consult, care, reminisce, celebrate and pray.Everything is done in complete confidence, no matter what your values, faith or beliefs may be.

You can contact us directly at the Martlets Hospice by telephone and ask to speak with a member of the Patient and Family Support team or let a member of the Multi-professional team know that you would like to speak with us and they can make a referral on your behalf. A member of the team will then contact you and arrange an initial appointment for you and your family.
