Referral Criteria

Introduction:

This document aims to set down the principles and priorities governing the admission of patients to Day Hospice.

Criteria for Referral to the Day Hospice:

• Patients need to be 18 years and over, living in the catchment area and consent to the referral.
• Referrals will be accepted from the District Nurses, Macmillan Teams, GP’s, and specialist teams. Patients are known to have an advanced cancer or other life limiting illness that requires specialist palliative care.
• The GSF Prognostic Indicator Guidance is used to determine the suitability of attendance at Day Hospice.*
• Patients may have difficult physical or psychological symptoms despite optimum tolerated therapy, and/or need psychological and emotional support, which may include planning end of life care issues.
• The referrer needs to consider the patient’s medical and physical condition to ensure safety of access to and from the patient’s home. The presence of severe confusion would prohibit admission to the Day Hospice.
• Patients are encouraged to use their own transport where possible. If patients are relying on Hospice transport, they need to be aware that they will have to stay for the duration of the day, 10am – 3.30pm. Patients also need to be aware that travelling time on the bus can take up to one hour.
• Patients need to take their own supply of drugs for their day at the Day Hospice.
• Patients will have ongoing assessments of their condition, including regular reviews whereby the appropriateness of the patient continuing at the Day Hospice, or being discharged, will be discussed.
